Medical Power of Attorney (MPOA) representation via Patient Partners LLC

  • Your medical power of attorney is your advocate and representative, chosen to enact your wishes regarding medical care in the event that you are unable to make those decisions for yourself. This role is also referred to as a healthcare proxy.

Advanced Healthcare Directive (AHD) completion or review

  • Identify what quality of life looks like to you. Based on our discussions together I will assist you in drafting your advanced healthcare directive which will serve as the basis for your chosen representative to make medical decisions on your behalf. This document for some is very sparse, but the more robust it is the easier it will be for your preferences to be carried out. Various disease processes have expected side effects and outcomes which often necessitates more explicit language and customization of your directive. Coming from a clinical background I can help you better understand these specific challenges and ensure your AHD is truly representative of your wants. Upon completion of your AHD it is advised that you review it with your chosen estate planning lawyer and have it notarized by a notary public. Your estate planning lawyer specializes in ensuring your assets are appropriately protected and your loved ones are taken care of. As such, they play a key role in getting everyone involved on the same page about who serves what role, be it in regards to financial or medical decisions. Your AHD is a living document, meaning that throughout different stages of your life we will sit down together and make adjustments to it’s components. The AHD put in place as a single person in their 20’s tends to look very different than that of a parent of three small children in their 40’s.

Legacy Project and Archival Work

  • A customized project that can be a lasting physical memento or something ephemeral.
  • Archival work is ideally a referral for Maureen and other collaborators. This can be in addition to or the entirety of said legacy project. Examples include: scanning and organizing photos into physical albums and digitized folders or websites for sharing, display or presenting physical mementos, reworking or repurposing heirlooms, journal/diary transcription, memoir writing, the workbook!, writing prompts including letters etc (can facilitate disposition of).
  • Park bench or other physical space (booth at a bar, plaque, brick etc), disposition of funds to support a charity or event in a revocable trust

Medical Aid in Dying Support 

  • Please keep in mind that the process from discussion to having a prescription in hand may take three to four weeks to complete, therefore timing is an important factor. Support includes finding providers that align with your values and are willing to prescribe should you qualify, completing the necessary paperwork with you, and in-person support during the dying process.

Disease Management

Pre and Post-operative Concierge Nursing

  • Individualized packages based on identified post-operative needs. I will work with your surgeon and discharging medical team to provide in-home follow-up and care. This includes facilitating communication with your healthcare team, ensuring you have any supplies and prescriptions needed, drain care and dressing changes.

Medication Management and Education

  • Perform medication reconciliation of all prescription and nonprescription medications, supplements, and remedies you are taking. This can be done in-person or remotely. Through this process you will be provided with directions for all medications and therapies, education on any side effects and important instructions, important phone numbers and contact information, monthly schedule of appointments and healthcare goals.

Transition support

  • During move to a higher level of care, i.e. home to SNF or assisted living etc, I am able to provide support during the transition by acting as an advocate to ensure appropriate medical histories and needs are communicated to new care team, help ease concerns of family, assist in settling into the new space, and providing continuity of care. This service can range from hourly (for new clients looking for strictly a short-term need) to packages and continued support services (create a medication management plan and binder, facility/home environment safety evaluation, assess for DME needs and gaps in services, family communication and support during disease progression).
  • This is about more than just losing weight or running a 5k. The goal is to give you the tools and support to improve your quality of life through symptom management. We will work together to track symptoms and monitor progression, identify behaviors that might be sabotaging progress and create new habits that you will be able to stick with long term.
